STEP 1: First of all, visit the UP Sects Portal at https://sects.up.gov.in/
STEP 2: At the homepage, click at “Apply for State Health Card” tab or directly click https://sects.up.gov.in/public/starttoapply.aspx
STEP 3: Upon clicking the link, the page to apply online for UP state health card will appear as shown below:-

STEP 4: Enter mobile number, captcha and click “Generate OTP” button to receive an OTP on entered mobile number. Validate the OTP, agree to terms & data security policy and click “Submit” button to open Sects UP Gov In card application form.

STEP 5: Fill in all the asked details accurately and click “Save and Next” button.
STEP 6: Add dependents (if any) and click “Submit and Print Application” button to forward your application to DDO/CTO for approval.

Pandit Deendayal Upadhyay Rajya Karmchari Cahless Chikitsa Yojana provides cashless treatment facility to UP State Government Employees, Retired State Government Employees and their dependents.

Cashless Treatment facility is available in Ayushman Empanelled Private and Government Hospitals.

Cashless Treatment up to Rs. 5 lakh per year per family is available to each beneficiary in Private Hospitals Empanelled under Ayushaman Bharat. Moreover, cashless treatment without any financial limit will be available to beneficiaries in Government Medical Colleges and Government Hospitals Empanelled with Ayushman Bharat.

All State Government Employees, Retired State Government Employees and their dependents are eligible for cashless treatment under the scheme and would be given a state health card.
